Hand from yesterday Who is to blame?

Who is to blame for reaching this slam?
Explanations:
East opened 1S as he felt the hand was too good for 1NT
2C = gf 2/1
2NT = waiting
3S sets S
3NT = serious
4H = LTTC

Second question, how do you make it on a trump lead? As it makes, is it a bad slam?

I blame East more. The minor suits aren't great for upgrading (but then again, I don't upgrade very often at all).

As for how to make it on a trump lead, win in dummy and play a diamond to the Q, then duck a diamond. Whatever North returns, win and draw trumps (must win the A if heart return), then cash the A, A and run trumps discarding clubs, North is squeezed on the last trump.

IMO, Bidding is not an exact Science and you both bid well to reach this poor slam.
Luckily, you can make it.. You can set up a club( or diamond) trick then Vienna coup North :)

I took the C hook and when that lost cashed the Ace of D and ran trumps. Interestingly, the datum was 460.

FWIW I would bid both hands the same way. Why is this a bad slam?

On a heart lead we are making if south has at least one honor (or on a high heart lead maybe we can reconsider).
On a diamond lead we are making if south led away from the K or just basically on a club hook (or K + long hearts if you want) if north wins.
On a club lead we are basically cold if he led away from the K (north should have the DK, and even if he doesn't we still have lots of chances) or if the red suit squeeze comes in if he didn't (the red suit squeeze is almost 50%).
On a trump lead we win and run the CQ, if it loses we just play for the red suit squeeze, and if it holds we can either just guess diamonds for 2 tricks or failing that, play for a red suit squeeze.
