That depends very much on the crowd you play with. Either penalty or takeout is reasonable and both are common.

The best indicator is that it is likely to be the same meaning as 1NT - (2H) - X.

Personally I don't like any double at this level to be penalty, but that's not to say it is in this rare(ish) auction. Any (sensible) player bidding 2M here is likely to have a six card suit (and opposite an opening bid that gives a reasonable chance of 2M being made.) So I would take it as takeout/competitive here, showing s and the values to compete to 2NT at least.

We play system-on after a 1NT overcall. This includes (for us) Take-out Doubles, Lebensohl and our SWINE run-out if doubled.

Like Tramticket, I think "system on" is best - bid as if your partner opened 1NT. However, Like FelicityR it is not penalty, and I play it as a transfer to the next suit up, either weak and 6 cards (more likely here) or strong. It is forcing, so you have the opportunity to bid again. If their bid is 2, it is a transfer to on the assumption that opener could be short.

I would think takeout showing exactly four spades, for most people.
