Looks like winning defense is to win A and lead 10. Easier to find if J denies a higher honor.

When West returns a , Declarer does best to cash both top s, according to deep finesse. Double dummy, he makes a miraculous overtrick :(

Declarer cashed only one top . Now, after west wins T, a small still defeats the contract.

winning the Da playing back a 2nd D is normal. who wouldnt?
playing the 3rd D appears to be mistake. one would already see that declarer would never get that D trick.

Like I said, playing back a diamond only looks right if partner led from KJ10(x)(x).

I led the diamond back because I couldn't see anything better. I wasn't able to picture the layout where giving declarer a free finesse in spades would be the killing defense.
