Easy to realise you're missing ♠KQ, particularly if your opening style is such that N's diamond suit need not be that good.
I would duplicate smerriman's auction except with another round of bidding between 2♠ and 4N to confirm partner actually had the 4th spade.
Trickier is where N's hand is xxxx, Axx, AQJxx, Q and you DO want to bid the slam in diamonds or NT but 5♠ may fail if the trumps don't break.

Good point about a 3 card raise - though if partner did bid 3NT, what then? Must still be a decent chance of slam.
